In this vital role, you will serve as a bridge between patients, families, and healthcare providers, ensuring seamless coordination of care and optimal patient outcomes. Your expertise will help navigate complex medical systems, facilitate discharge planning, and provide culturally sensitive support to Spanish-speaking populations. At this time, we are seeking to hire case managers for all boroughs (Brooklyn and Queens).
Duties
- Conduct thorough patient intake assessments in both English and Spanish to gather medical history, social determinants, and care needs
- Coordinate discharge planning by collaborating with multidisciplinary teams, including social workers, nurses, physicians, and community resources
- Manage utilization review and utilization management processes to ensure appropriate level of care and resource allocation
- Document clinical encounters accurately using Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems while maintaining HIPAA compliance
- Assist with case management activities across various specialties
- Support patients through behavioral health or hospice care transitions by providing culturally competent guidance and emotional support
- Facilitate communication between healthcare providers and patients in both languages to ensure understanding of treatment plans and follow-up instructions
- Create a Plan of Care to address client's needs
- Medication Management
- Link clients and families to crisis counseling, therapeutic services, and safety interventions
- Assists with applying/obtaining benefits/entitlements (HRA benefits, SNAP, WIC, Social Security, Food Pantry, Access-A-Ride, etc.)
- Maintaining weekly contact via telecommunications with the client and their family to address their needs
Experience
- Strong background in nursing, social work, or case management within managed care or utilization review environments
- Demonstrated ability to communicate fluently in both English and Spanish with diverse patient populations
- Understanding of HIPAA regulations and commitment to maintaining patient confidentiality at all times
Required:
- Bachelor Degree in Social Work, Psychology, Sociology, Human Services and/or related field (with 2 years experience is a plus) or
- Master's Degree in related field (with 1-year experience is a plus)
- CANS-NY certified upon hire(Required)
- Spanish speaking (Preferred)
