We do not discriminate on the basis of race, religion, color, sex, sexual orientation, age, national origin or disability.Summary of Duties:The Resident Services Case Manager will be part of LTSCs Resident Services Department, providing comprehensive case management support to residents of Umeya, a 175-unit deeply affordable and permanent supportive housing community located at the intersection of Little Tokyo and Skid Row. This position focuses on serving residents in Umeyas general affordable units through direct outreach, individualized case management, job-readiness coaching, childcare referrals, and connection to community resources.We are seeking a compassionate individual with experience supporting individuals and families through a trauma-informed lens.On a day-to-day basis, the Case Manager will conduct regular outreach to residents, build trusting relationships, and collaborate with community partners to foster a supportive environment and strengthen access to services. The Case Manager will provide case management support including intake and assessment, goal setting, progress monitoring and reassessment, life-skills development, supportive counseling, benefits assistance, and referrals to appropriate resources.The primary goal of this roles is to help residents achieve long-term stability by addressing immediate needs while supporting progress toward greater self-sufficiency.Additionally, the Case Manager will work closely with the Umeya Resident Services Coordinator, the Director of Resident Services, as well as the other members of the Resident Services team, Community Organizing, Social Services, and Small Business Counseling teams to strengthen relationships with local stakeholders, including small businesses, institutions, and residents in Little Tokyo, to create meaningful community-building and engagement opportunities for Umeya residents.Responsibilities include: Outreach & EngagementConduct regular outreach to residents in Umeyas general affordable unitsBuild trusting, culturally responsive, and trauma-informed relationships with residentsMaintain a consistent presence in the building to increase visibility and accessibilityEngage residents in community events, workshops, and onsite programmingCase Management & Supportive ServicesComplete intakes, assessments, and individualized service plansSupport residents in identifying goals related to housing stability, employment, health, education, and financial well-beingAssist residents with applications for public benefits, services, and entitlementsCoordinate referrals to healthcare, mental health services, childcare, transportation assistance, workforce programs, and other community resourcesHelp residents navigate crises and connect them to appropriate crisis intervention supportsServe as an advocate for residents in interactions with property management and service providers when appropriateCommunity Building, Engagement & Collaboration(Working closely with the Umeya Resident Services Coordinator)Coordinate and support community events, workshops, and resident engagement activitiesFoster a welcoming environment that promotes safety, connection, and community cohesionBuild partnerships with Little Tokyo and Skid Row stakeholders, small businesses, institutions, community groups, to create opportunities for residentsCoordinate services with onsite property management to address resident needs while maintaining appropriate boundariesMaintain strong professional relationships with external partners and resource providersDocumentation & ComplianceMaintain accurate, timely, and confidential case notes and records in accordance with agency standardsTrack resident progress, service outcomes, and program metrics through ApricotPrepare reports as required by internal policies or funding sourcesFollow all ethical standards, privacy regulations, and organizational policiesAdditional DutiesAssist in developing new programs or initiatives to meet emerging resident needsParticipate in trainings to strengthen trauma-informed, culturally responsive, and housing-focused practicesProvide general support within the Resident Services team and contribute to departmental planning and evaluationOther duties as may be assigned by your supervisor*Execution of some of the above duties may require driving or use of public transportation to provide services at satellite office or in the field.Qualifications, Experience, and Skills:Bachelors degree in Social Work, Mental Health, Human Services, or a related field required.Experience in providing case management or outreach services to vulnerable populations (both professional and/or volunteer experience is sufficient)Any equivalent combination of education, experience, or training that has prepared you for the roleStrong understanding of affordable housing, resident services, case management practices, and culturally responsive, trauma-informed service delivery, with knowledge of community resource networks.Strong communication, problem-solving, and conflict resolution skills, with professionalism, discretion, and emotional intelligence in engaging residents, staff, partners, and stakeholders.Proficiency in computer applications, including word processing, spreadsheets, and internet researchStrong organizational and planning skills; Strong record-keeping skills for tracking metrics and participationBilingual ability (English/Spanish) preferred.Equivalent education or experience can be substituted for all minimum qualifications, except when legal requirements, such as a license/certification/registration, are required.
