Description

As the QIDP Instruction and Supports Manager, you will be responsible for ensuring that appropriate, individualized, high-quality support services are delivered to individuals following the standards set by the DHS. Additionally, you will provide training and retraining to division staff and lead the referral and intake process for the division. 


Rates: $50,000 - $55,000/Year

Schedule: Monday-Friday 7:30a - 3:30p

SPECIFIC DUTIES:

1.   Acts as resource to all division staff regarding problematic instructional situations including, but not limited to, life skills, daily living activities, and behavioral situations.

2.   Attends conferences, meetings, and visits other programs to gain knowledge of various instructional methods, current best practices, and emerging procedures. Effectively relays relevant information to staff.

3.   Becomes familiar with materials used for instructing the individuals (TEACHH, PECS, SCIL and Murdoch curricula, DT-developed training programs) and the methods used to implement training programs (shaping, backward and forward chaining, etc.)

4.   Coordinates and records all required Little Friends and On the Job training for division employees – including annual requirements.

5.   Works cooperatively with the division management and leadership team to assist staff with goal writing, tracking, and implementation, and the development of the Implementation Strategy for individuals. 

6.   Ensuring that appropriate, individualized, high-quality support services are delivered to individuals on caseload, to include but not limited to:

       a. Ensuring there are adequate levels of staff support for those individuals on their caseloads. To include:

            i. Ensuring that individuals on caseload are assigned to DSPs as appropriate.

            ii. Ensuring coverage of DSPs when these positions are open or staff is absent.

       b. Directly observing and working with assigned staff in work units.

7. Participating in meetings for all individuals on their caseload, following established procedures to complete documentation thoroughly, accurately and without delay. To include but not to:

       a. Participating in Discovery meetings and Personal Plan meetings as requested by families  and ISC agencies.

       b. Ensuring that rooms are booked for Personal Plan or Discovery meetings as requested.

       c. Ensuring that supervised staff attend meetings as requested and bring with them all pertinent reports and documentation, prepared in legible, thorough and accurate fashion.

       d. Complete monthly Q-notes.

8.   Ensuring that reports/Implementation Strategies are completed according to DHS regulations. This including but not limited to, completion in required time frame, ensuring that all required signatures are acquired, outcomes are presented to DSPs and they are appropriately trained on supporting the individual to obtain those desired outcomes.

9.   Ensuring that annual release packets are completed and filed for all individuals on caseload.

10.   When needed, supervises individuals throughout programming to ensure safety on site or in the community. 

11.   Maintains verbal and written contact with families/guardians of assigned individuals as necessary to promote growth in training areas.

12.   Maintains verbal and written contact with CILA staff to promote harmonious coordination of services, update each other on individuals’ progress and promote growth in all areas of the individuals’ lives.

13.   Recommends materials for purchase to leadership for use in the program.

14.   Fills out Maintenance Request form in FamCare upon observing a maintenance need in the building.

15.   Is aware of individual’s medical problems as they relate to performance or safety. Notes changes in individual’s medical/physical status and reports these in writing as needed to families, residential staff and division leadership.

16.   Physically assists individuals weighing 150 lbs. using the toilet to include but not limited to: 

         a. Transferring individual from wheelchair to toilet and visa versa, helping individual to dress/undress and ensure individual has cleansed themselves properly.

         b. Will also physically assist individuals who require additional assistance after toileting is completed to include but not limited to: cleansing themselves, after toileting accidents and hand washing.

17.   Physically assists individuals in personal care such in brushing teeth and washing hands and faces as requested.

18.   Ensures that each individual takes the proper transportation home each day.

19.   Participates in Strategic Planning sessions for division as requested.

20.   Working cooperatively with division leadership to identify applicants who qualify for the program and moving them into the program when openings occur. This includes but is not limited to:

         a. Working with the agency family care team to properly track referrals and interested parties in FamCare.

         b. Coordinating initial qualification process for clients, including but not limited to: arranging for tours, ensuring receipt of necessary intake information (i.e. funding award letter, guardianship paperwork etc.),  working with other division staff on appropriate placement for assessment, and all other tasks necessary for entry into the program.

         c. Ensures that enrollment is maintained at maximal levels for CDS Medicaid Waiver program.

21.   Serves on other teams and standing committees of division, as assigned.

Other duties as assigned by Director of Adult Day Services. 

Requirements

  

1.   Must be capable of reading, writing, and speaking English to plan, coordinate, and schedule programming with program staff and individuals, guardians/caregivers (when necessary), other agency staff, and stakeholders. Written communication must be clear, concise, grammatically correct, and free of spelling errors.

2.   Must possess sufficient strength, agility, and sensory acuity to support program participants safely and effectively during programming hours. This includes responding to behavioral problems in which the individual is displaying aggression towards themselves or others, or other emergencies using learned safety measures, as well as assisting with daily living tasks (such as toileting or dressing) with individuals weighing 150 lbs.

3.   Must behave in a professional manner that reflects positively on themself and Little Friends Inc. This includes demonstrating high commitment to customer service and customer satisfaction for all internal and external customers of Little Friends Inc.

4.   Must possess sufficient computer skills for training, intake, and record keeping. 

5.   Must be aware of, and follow, the policies and procedures of Little Friends Inc. and Adult Day and Community Day Services. 

6.   Is required to complete all training and recertifications as per Little Friends, ADS/CDS, and Department of Human Services

7.  Must be able to regularly lift 50lbs

8.  Must possess a valid IL driver's License 

9.  Must possess a Bachelors degree or greater in a QIDP-qualifiable major

10. Must possess a minimum of 1 full year of experience working with individuals with developmental disabilities
