Salary: $81,681–$94,534/year (DOE)

💼 Position Summary
The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), Care Area Assessments (CAA), and care plans in accordance with state and federal regulations. This role plays a key part in ensuring quality outcomes, accurate reimbursement, and a positive resident experience.
✅ Key Responsibilities
- Plan, schedule, and complete MDS assessments and submissions
- Lead interdisciplinary care planning and documentation review
- Identify and address changes in resident conditions
- Monitor Medicare eligibility and documentation
- Ensure compliance with state and federal guidelines
- Support Quality Assurance and Performance Improvement (QAPI) efforts
- Educate and support clinical staff on the MDS process
