TCM -Mental Health caseworker

Job Overview
The Case Manager plays a crucial role in coordinating patient care and ensuring that individuals receive the necessary services to support their health and well-being. This position involves working closely with youth in Foster care and their, families, and healthcare teams to assess needs, develop care plans, and facilitate access to appropriate resources. The ideal candidate will possess a strong understanding of trauma informed care and caae management and crisis intervention.

Duties

• Conduct comprehensive assessments of youth needs and develop individualized care plans.
• Collaborate with healthcare providers to coordinate services and ensure continuity of care.
• Facilitate discharge planning f transitioning from I grouphome settings to home or other care facilities.
• Provide Community connections
• Maintain accurate medical records and documentation in compliance with DCF regulations.
• Educate youth and families about available resources, treatment options, and self-care strategies.
• Monitor patient progress and adjust care plans as necessary to meet evolving needs.
• Advocate for clients rights and ensure they receive appropriate services.

Skills

• Proficiency in emergency medicine principles and practices.
• Experience in inpatient settings, particularly within acute care environments such as Grouphomes.
• Strong knowledge of medical terminology relevant to patient care and case management.
• Ability to navigate medical records systems effectively.
• Excellent communication skills for patient service and collaboration with multidisciplinary teams.
• Familiarity with discharge planning
