Job Summary:
We're seeking a compassionate and skilled LPN to join our team of healthcare professionals in Raleigh, NC. Responsibilities:
Provide direct patient care, including administering medications, monitoring vital signs, and performing treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct vital sign checks, report any changes or concerns to the nursing team, and document patient information accurately and thoroughly
Assist with daily living activities, such as bathing, dressing, and grooming, as needed
Participate in quality improvement initiatives and contribute to a positive and respectful work environment
Maintain patient confidentiality and adhere to HIPAA guidelines
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
Requirements:
Current LPN licensure in the State of North Carolina
High school diploma or equivalent required
