Job Summary
We are seeking a dedicated and compassionate Community Habilitation Specialist to empower an individual with developmental and cognitive disabilities to achieve greater independence and improve their quality of life. In this vital role, you will provide personalized support, facilitate skill development, and foster community engagement for an individual with a Traumatic Brain Injury. Responsibilities
- Develop and implement individualized habilitation plans tailored to this women's unique abilities, goals, and needs.
- Assist individual with activities of daily living (ADLs), including personal hygiene, meal preparation, and household tasks.
- Support client during community outings, transportation, and social activities to promote independence and community integration.
- Collect data daily on client progress, document observations, and maintain accurate records for ongoing assessment and reporting to help master their desired outcomes.
- Communicate regularly with clients, families, caregivers, and interdisciplinary teams to ensure coordinated care.
- Supervise medication or first aid as needed following established protocols and safety guidelines.
Qualifications
- Experience working with individuals with disabilites
- Desire to work with a service dog recommended
- Prior experience in home & community care settings or social services environments preferred or educational background in this field
- Familiarity with assisting clients with cognitive disabilities and supporting ADLs. Driving license and transportation for local travel is necessary.
- Strong caregiving skills combined with patience, empathy, and the ability to add to the individual's Life Plan desired
Pay: $25.00 - $30.00 per hour
Expected hours: 10.0 – 15.0 per week
