About The Role
- Complete strengths-based assessments and care planning
- Undertake safeguarding enquiries and risk assessments
- Manage a varied Adults Social Work caseload
- Work collaboratively with health professionals, families, and partner agencies
- Support adults to maximise independence and achieve positive outcomes
- Maintain accurate case records and reports
Requirements of the Role
- Qualified Social Worker
- Social Care Wales registration
- Experience within Adults Services
- Strong assessment, communication, and decision-making skills
- Full UK driving licence and access to a vehicle
