Position Summary

The Foundational Community Supports (FCS) Specialist provides Medicaid-funded Supportive Housing and/or Supported Employment services to eligible Apple Health members enrolled in Washington State's FCS program.

The Specialist works directly with individuals experiencing complex health, behavioral health, housing, and employment barriers. Services are delivered in the community using recovery-oriented, trauma-informed, culturally responsive, and person-centered approaches.

The ideal candidate enjoys working independently, building strong community partnerships, and making a meaningful impact in the lives of vulnerable populations.

Essential Responsibilities

Supportive Housing Services

  • Conduct person-centered assessments and develop individualized service plans.
  • Assist participants in locating and securing safe, affordable, and permanent housing.
  • Support applications for housing programs, rental assistance, and benefits.
  • Coordinate with landlords, housing authorities, and community organizations.
  • Teach independent living skills, budgeting, tenancy responsibilities, and lease compliance.
  • Help participants maintain stable housing through ongoing support and crisis intervention.
  • Conduct home visits and community outreach.

Supported Employment Services

  • Assist participants in identifying career goals based on their strengths and preferences.
  • Provide resume development, interview preparation, and job search assistance.
  • Develop relationships with local employers.
  • Support participants with job placement and job retention.
  • Provide on-the-job coaching and follow-up support as needed.
  • Coordinate employment services with treatment teams and vocational partners.

Documentation & Compliance

  • Complete timely and accurate Medicaid-compliant documentation.
  • Maintain participant records in the agency's electronic health record.
  • Complete assessments, progress notes, service plans, referrals, and discharge summaries within required timelines.
  • Ensure compliance with Washington Health Care Authority (HCA), HIPAA, and agency policies.
  • Participate in audits, quality assurance activities, and supervision.

Qualifications

Required

· High school diploma or equivalent

· Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, or a related field.

o Equivalent education and relevant experience may be considered.

  • Minimum one (1) year of experience working with adults experiencing behavioral health conditions, homelessness, substance use disorders, or other complex needs.
  • Valid Washington State Driver's License and reliable transportation.
  • Ability to travel throughout assigned service areas.
  • Strong computer skills and experience with electronic documentation.
  • Excellent communication and organizational skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.

Preferred

  • Experience providing Washington State Foundational Community Supports (FCS) services.
  • Knowledge of Medicaid documentation requirements.
  • Familiarity with Apple Health, HCA programs, and community resources.
  • Experience with Supportive Housing or Supported Employment.
  • Experience with DAP documentation standards
  • Bilingual candidates are strongly encouraged to apply.

Pay: $23.00 - $27.00 per hour
