Description
- Conduct comprehensive assessments of patients' physical abilities and limitations.
- Develop individualized care plans tailored to the specific needs and goals of each patient.
- Administer effective physical therapy treatments and interventions.
- Monitor and document patients' progress, adjusting treatment plans as necessary.
- Educate patients and their families on proper exercises, techniques, and strategies for continued improvement.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated and holistic care.
- Provide guidance and support to caregivers and family members on safe and effective patient care techniques.
- Maintain accurate and up-to-date patient records and documentation in compliance with regulatory standards.
- Stay abreast of current developments in physical therapy practices and incorporate evidence-based techniques into treatment plans.

Qualifications
- Valid state licensure as a Physical Therapist.
- Two (2) years of experience in physical therapy, with a preference for experience in home care or a related setting.
- Strong clinical assessment and diagnostic skills.
- Excellent interpersonal and communication skills, with the ability to establish rapport with patients and their families.
- Demonstrated proficiency in developing and implementing personalized care plans.
- Knowledge of relevant legal and regulatory standards related to home care physical therapy.
- CPR certification and other relevant certifications may be required.
Pay rates based on visit-based compensation structure!
