Join our team at Lincolnwood Rehabilitation and Health Center as a Social Worker.Proudly supported by Marquis Health Consulting Services Part-time opportunity available24 hours weekly $25-$50 an hour (all inclusive)Responsibilities of Social Worker:Evaluate patients (existing, new and prospective) based on their needs and limitationsAddress patient concerns and goals while maintaining constant communication with the patient as well as a case managerBuild rapport with patients and their families Coordinate care with PT departments Develop care plans for patients Qualifications of Social Worker:Must be able to work full-time hours and be flexible Bachelor’s degree required. Master’s degree is strongly preferred.Minimum three (1) years' experience as a social worker in a healthcare-related setting.
