SUMMARY:

The Licensed Practical Nurse is a member of the Treatment Team. S/he is responsible for coordinating medical care for program participants and reports directly to the Director of Nursing. This is an hourly position with hours and schedule established to meet patient needs; weekend hours may be required.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Ensures that general health assessments are completed in a timely fashion for all patients.
  2. Facilitates referrals to physicians, dentists, and other health care professionals as indicated by the assessed needs of each patient.
  3. Provides assistance to personnel in determining the appropriateness for admissions as needed.
  4. Participates in clinical staffing meetings.
  5. Conducts groups and other educational groups as necessary.
  6. Maintains appropriate patient health records.
  7. Interacts with community agencies and refers patients to facilitate their health maintenance.
  8. Delivers patient care in accordance with medication protocols, detoxification protocols, and drug screening procedures.
  9. Is available to other staff to address / answer concerns, provide after-hour directives and help problem solve patient related issues.
  10. Insures the flow of information both horizontally and vertically within the organization.
  11. Provides appropriate documentation for contract and system requirements and clinical best practice.
  12. Makes sound, independent, cooperative, and ethical decisions.
  13. Monitors, reconciles, and administers medications safely and efficiently.  

 

ADDITIONAL DUTIES AND RESPONSIBILITIES: 

Performs other duties as deemed appropriate by supervisor.

 

MINIMUM QUALIFICATIONS:

  1. The Licensed Practical Nurse must have a minimum of a current LA Licensed Practical Nurse (LPN) license.

 

ESSENTIAL KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:

  1. Must demonstrate the ability to effectively function as a LNVR team member.
  2. The ability to effectively manage multiple tasks concurrently.
  3. Must possess and demonstrate effective communication and organizational skills.
  4. Must demonstrate the ability to exercise discretion and independent judgment customarily and regularly in significant matters.
  5. Must demonstrate the ability to relate with empathy and sensitivity to staff, patients, and the community.

 

COMPETENCIES:

  1. Demonstrates the ability to systemically collect, record, and analyze data and other relevant interactive medical and behavioral systems to improve patient care. 
  2. Demonstrates the ability to accurately screen for the early detection of risk of imminent harm (suicide and self-harm) to self or others (aggressive or violent behaviors including homicidal ideation) and provides the need for immediate intervention according to organization policy, procedure and/or protocols.  
  3. Appropriately utilizes COWS/CIWAS and Pain scales in accordance with prescriber orders to assess opioid and alcohol withdrawal and physical pain to determine optimal timing of induction dose of partial agonist or symptom reducing medication.

  2. Provides effective and proficient warm hand off communication with behavioral health providers and other interdisciplinary team members to uphold continuity of patient care. 
  3. Identifies and participates in quality improvement initiatives to help improve the overall safety and quality of patient care.
  1. Participate in continuing professional development activities to maintain competency in area/s of practice.
  1. Obtains all specimens, including urine specimens, in a trauma- informed and culturally sensitive manner to respect the dignity and privacy of all patients.
  2. Uses clear, concise, no stigmatizing and medically appropriate language when communicating with patients, providers, and in the electronic medical record.
  3. Demonstrates culture of safety behaviors as one that is open, possesses mutual respect when discussing or presented with safety concerns, provides solutions without shifting to individual blame, and establishes a learning environment with transparency and accountability among the treatment team.
