Job Title: Lead Medical Coordinator  

 

Immediate Supervisor: Director of Nursing 

 

Department: Health Center 

 

Immediate Subordinates: Medical Coordinators 

 

Program Overview 

 

Lincoln Hall’s Family Reunification Program (FRP) is on a 450-acre campus in Westchester County, NY, about 45 minutes north of New York City. The FRP provides temporary shelter services to unaccompanied children until they can be safely reunified with sponsors in the United States. While in our care, the children receive a host of services, including clinical, case management, medical, nutritional, recreational, religious, and educational services, all aimed at supporting the child’s physical, mental, behavioral, and emotional well-being. 

 

Position Description 

 

The Nurse Medical Coordinator (NMC) is a licensed registered nurse (RN) responsible for overseeing and coordinating the comprehensive care of unaccompanied children (UC) in the care of the Office of Refugee Resettlement (ORR). The NMC ensures that high quality medical services are delivered in compliance with ORR policies, state licensing requirements, and federal regulations.     


Essential Functions 

  • Coordinate and monitor ongoing medical, dental, and mental health care for all UC in the program. 
  • Ensure compliance with ORR medical policies and procedures, including maintaining accurate and timely documentation in the electronic health record (EHR) system (e.g., UAC Portal). 
  • Collaborate with shelter staff, case managers, mental health clinicians, and external healthcare providers to ensure continuity and quality of care. 
  • Develop individualized care plans in collaboration with patients, families, physicians, and interdisciplinary team members as well as coordinate services across the continuum of care.  
  • Monitor and evaluate patient progress, modifying care plans necessary to achieve desired outcomes. 
  • Advocate for patient needs, ensuring access to appropriate healthcare resources, medications, and support services
  • Acts as a liaison between the infirmary and other departments.
  • Administer medications and maintain proper medication documentation and storage. 
  • Track and ensure timely completion of immunizations and routine health screenings per ORR standards. 
  • Prepare for and participate in audits and reviews by ORR, state licensing, and other governing bodies
  • Facilitate transitions of care, including discharge planning, to ensure continuity and supported and safe community adaptation 
  • Provides direct and indirect care given to the residents in compliance with agency policies, professional standards, scope of practice, practitioner’s orders, and state and federal guidelines.
  • Participates in the admission process. Nursing assessment is based on interview, examination, observation and review of records of the resident.
  • Provides nursing assessment, triage, first aid and treatment to residents seen in the clinic for routine and emergency care.
  • Provide oversight and/or administration of vaccinations in accordance with the American Academy of Pediatrics.
  • Transcribes medical orders. Processes medication from pharmacy, administers and documents medications and treatments.
  • Always maintains the required level of professionalism in practice at the agency and abides by the HIPAA Privacy Act and Confidentiality Policy of the facility.

 

 

Position Requirements 

  • Associate's or Bachelor’s degree in Nursing (ASN or BSN) from an accredited nursing program. 
  • Bilingual (English/Spanish) strongly preferred. 
  • Current, unrestricted RN or LVN/LPN license in the state of employment. 
  • Minimum of 1–2 years of nursing experience, preferably in pediatrics, public health, community health, or a shelter/residential setting. 
  • Experience working with immigrant or refugee populations, preferred. 
  • Current CPR and First Aid certification. 
  • Ability to pass a federal background check and child abuse/neglect registry check. 
  • Familiarity with trauma-informed care practices. 
  • Understanding of cultural sensitivity and the ability to work with diverse populations

 

Salary: $105,000.00 

Job Type: Full Time/Non-Exempt 

Shift: standard working hours 

 

Working Conditions 

  • Work is performed in a shelter or residential facility for unaccompanied minors. 
  • May involve evening, weekend, or on-call hours depending on program needs. 
  • Exposure to communicable diseases and potentially emotionally challenging situations.
