What are the responsibilities and job description for the Houseless Outreach and Resource Navigator position at Lincoln County?
Lincoln County Health and Human Services is looking to add a Houseless Outreach and Resource Navigator to their team to assist with their outreach efforts to some of our most vulnerable citizens. The person in this role will assess the physical health, mental health, and social support needs of unhoused individuals supported by the Lincoln County Shelter Program. Provide services to those supported in the Emergency Winter Shelters and those identified through outreach efforts.
This position is also responsible for providing navigational support for identified individuals (i.e., linkage, monitoring, advocacy, documentation), as needed to help connect people with the resources they need and can most benefit from.
Job Conditions: Normal working conditions. May be encountering unstable clients in their homes or community settings.
This position works 12:00 p.m. to 8:00 p.m. October 1st through March 31st, and 8:00 a.m. to 5:00 p.m. April 1st through September 30th.
Work Location: Lincoln County
Travel: Travel regularly in county vehicle with clients.

Minimum Education Level
High School Diploma / GED
Minimum Experience Level
Two- Three years experience in a Health and Human Services setting working with marginalized or vulnerable populations.
Certifications
Must be able to obtain OHP Assistance Certification within six months of employment.
Driver’s License
Oregon Driver's License required within 30 days of hire.
Desirable Qualifications: Bilingual and/or bicultural competencies for the Latino population are preferred.
Knowledge of:
- Needs of underserved and vulnerable populations.
- Health disparities and trauma informed service delivery.
- Basic HHS programs, community programs, and services available.
- Eligibility and enrollment rules and procedures and coverage options under Medicaid, OHP, or the Health Care Market Place.
