What are the responsibilities and job description for the Houseless Outreach and Resource Navigator position at Lincoln County, OR?
Position Summary
This position is also responsible for providing navigational support for identified individuals (i.e., linkage, monitoring, advocacy, documentation), as needed to help connect people with the resources they need and can most benefit from.
Job Conditions: Normal working conditions. May be encountering unstable clients in their homes or community settings.
Work Location: Lincoln County
Travel: Travel regularly in county vehicle with clients.
Essential Functions/Major Responsibilities
- Screen unhoused individuals for the social determinants of health, using tools identified by the HHS Department.
- Determine health and social support needs.
- Develop plans to address resources and support needs.
- Document findings according to procedures established by the County HHS Department.
Linkage and Monitoring:
- Carry out the traditional functions associated with service navigation to include linkage, monitoring, and advocacy.
- Provide coaching utilizing motivational interviewing strategies to encourage individuals to register for and accept available services and supports.
- Assist individuals with Medicaid/Oregon Health Plan coverage application.
- Link/refer individuals to needed services using electronic platforms determined by the County HHS Department.
- Monitor, track, and report on the status and progress of identified individuals.
- Respond to referrals from health care providers or community partners to support people with economic, logistical, and other non-clinical barriers to accessing care.
Outreach:
- Assist in development and presentation of educational material for clients.
- Outreach by phone, online, or in-person to identify unhoused individuals who are not currently supported by existing resources.
- Participate in the annual Point in Time Count.
Other functions:
- Maintain division productivity level standard as established for this position.
- Provide specialized services as needed; assist in providing training to new employees.
- Attend team meetings, participate in quality improvement projects, and attend trainings as needed.
- Collect data, as directed, that is relevant to the operation of shelter and outreach efforts.

Desirable Qualifications: Bilingual and/or bicultural competencies for the Latino population are preferred.
Knowledge of:
- Needs of underserved and vulnerable populations.
- Health disparities and trauma informed service delivery.
- Basic HHS programs, community programs, and services available.
- Eligibility and enrollment rules and procedures and coverage options under Medicaid, OHP, or the Health Care Market Place.

Lincoln County offers a comprehensive benefit package including vacation, holiday, and sick leave as well as medical, dental, vision which is paid at 90% by the County and 10% by the employee. The County's current contribution rate is 11%.
Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
