What are the responsibilities and job description for the RN Case Manager/Care Navigator position at Lincoln County Health System?

- Monday - Friday 7:00 a.m. - 3:30 p.m.
- Every third weekend call rotation
Utilization review, collaboration with patients/families to facilitate the process of hospital care in an efficient and cost effective manner. Monitors level of care while navigating patient's progression of care through hospitalization. Serve as advocate for patient and family. RN with BSN preferred, minimum of 5 years nursing experience, case management or ICU experience preferred.
