Leading Into New Communities, Inc. (LINC) is seeking a compassionate and community-focused Community Health Worker (CHW) to provide outreach, engagement, and supportive services to justice-involved individuals participating in the Pathways to Community Program funded by the Governor’s Crime Commission (GCC). This role serves as a bridge between participants and critical community resources, helping individuals navigate behavioral health, substance use recovery, housing, employment, and reentry services.
This position is part of a grant-funded program. Employment is expected to continue through September 2027; however, continued employment is contingent upon grant funding and is not guaranteed beyond the life of the grant.
Key Responsibilities:
- Conduct proactive outreach and engagement with high-risk individuals in the community
- Build trusting relationships and identify early warning signs of crisis or instability
- Provide trauma-informed crisis intervention and de-escalation support
- Connect participants to behavioral health, substance use treatment, housing, and employment resources
- Assist with safety planning, intake, screenings, and individualized service plans
- Support participants in navigating systems of care and reducing barriers to services
- Maintain regular participant contact to encourage engagement and retention
- Document services accurately and collaborate with Case Managers and community partners
- Support group activities and Cognitive Behavioral Interventions (CBI), as assigned
Qualifications:
- High school diploma or equivalent required
- Associate’s or Bachelor’s degree in a human services field preferred
- Community Health Worker and/or Peer Support Specialist certification preferred or willingness to obtain certification
- Experience working with justice-involved individuals, underserved populations, or individuals with behavioral health/substance use challenges preferred
- Strong ability to connect individuals to community resources and navigate service systems
