Responsibilies
*Coordinate MDS and Care Plan process
*Instrument (RAI) guidelines and Medicare/Medicaid reimbursement models (PDPM)
*Ability to evaluate residents needs and develop care plans
Requirements
*Valid RN license in the state of practice
* 2-3 years experience in a long-term care setting
* 1 years MDS experience
*Comprehensive knowledge of MDS and resident assessment
* Strong knowledge of infection control practices and critical care procedures.
* Excellent communication skills for effective interaction with patients and families.
* Ability to work collaboratively with a healthcare team.
*Attention to detail
* BLS Certification
