Lilycare Hospice is hiring a Hospice Director of Nursing (DON) to lead clinical operations for our growing community-based hospice in Bedford, New Hampshire. You'll run the hospice IDT, build the nursing team, and own every clinical decision — while a dedicated Hospice Administrator handles business operations, HR, and finance separately.
$120,000–$140,000/year. You lead hospice clinical care. They handle the rest.

Duties and Responsibilities
As Hospice Director of Nursing (DON), lead the Interdisciplinary Team and serve as the clinical bridge between field hospice staff, the Medical Director, and agency leadership
Build and manage the hospice on-call rotation; participate in on-call coverage and step into the field to back up case managers during high census or staff shortages
Recruit, hire, train, and evaluate all hospice clinical staff — RNs, LPNs, CNAs; conduct supervisory visits, joint visits, and annual clinical evaluations
Run weekly IDT meetings; oversee hospice admissions, Plans of Care, certifications, recertifications, and symptom management; triage clinical questions from case managers in the field
Ensure compliance with Medicare Hospice Conditions of Participation and New Hampshire state hospice regulations; lead QAPI, formulary oversight, and clinical documentation standards
Compensation and Schedule
Pay range: $120,000–$140,000/year
Full-time, 40 hours/week
Monday–Friday on-call rotation required

Skills and Qualifications
Active RN license in New Hampshire or NLC multistate compact (required)
Graduate of an accredited nursing program
Minimum 1 year of administrative or supervisory experience in hospice, home health, or community health (required)
Strong clinical background in hospice and/or home health
Working knowledge of Medicare Hospice Conditions of Participation
Excellent communication skills with the ability to work effectively across hospice physicians, clinical staff, patients, families, and community partners
Valid driver's license and reliable transportation
BSN (preferred)
Prior Hospice Director of Nursing (DON) or Clinical Director experience (preferred)
CHPN certification (preferred)
