Position Summary:We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our Adult Day Care team. This role focuses on conducting high-quality clinical assessments, developing individualized care plans, and ensuring ongoing compliance through the New York State Uniform Assessment System (UAS-NY). The RN works closely with residents, families, and interdisciplinary care teams to promote optimal outcomes and continuity of care.Responsibilities:Perform in-person clinical assessments for existing residents in accordance with UAS-NY standards, agency policies, and state regulationsComplete UAS assessments to determine eligibility and continued participation in servicesEvaluate prospective residents by assessing clinical, social, and environmental needs to determine appropriate levels of careDevelop and maintain individualized service plansGather and analyze clinical data from multiple sources to support sound care decisionsCommunicate effectively with residents, families, healthcare providers, and internal teams to ensure assessment accuracyApply clinical judgment when supporting individuals with chronic or complex bio-psychosocial needsEnsure all documentation meets regulatory and organizational compliance standardsQualifications:Active Registered Nurse (RN) licenseUAS-NY Certification requiredExperience or strong familiarity with UAS-NY preferredPrior experience in MLTC or Managed Medicaid environments is a plusExcellent communication, organizational, and interpersonal skillsAbility to work independently and manage time effectivelyStrong computer proficiency and fluent English communicationShift:Full TimeDay schedule (details discussed during interview)EMR:UAS-NY systemN/P Ratio:Not applicable – Assessment-based roleFloat:No floating requiredCompany DescriptionLila Medlink is a dynamic, Physician-led, and veteran-owned staffing agency committed to connecting healthcare organizations with top-tier talent.
