As a Care Team Coordinator, you’ll play a central role in ensuring our caregivers and clients receive top-quality support. You’ll manage caregiver onboarding, training, scheduling, and evaluation, while providing exceptional service and communication to clients, caregivers, and referral sources.

• Coordinate with the Talent Acquisition and Recruiting teams to onboard new caregivers.
• Oversee caregiver onboarding, orientation, training, performance evaluations, and ongoing career development.
• Work closely with the Care Manager to schedule caregivers appropriately and manage assignments.
• Handle scheduling changes, call-offs, and no-shows with professionalism and urgency.
• Build and maintain strong relationships with clients, caregivers, and referral partners.
• Conduct regular “close to the client” visits and follow-ups to ensure outstanding service and satisfaction.
• Support service recovery and client retention through excellent communication and problem-solving.

• A people person with strong communication and interpersonal skills.
• Highly organized, detail-oriented, and able to juggle multiple priorities.
• Professional and assertive, with sound judgment and a caring attitude.
• Experience in home care, healthcare, or staff scheduling preferred.
• Ability to work collaboratively within a supportive office environment dedicated to quality care.
