Location: Oakmont, Zelienople or Canonsburg, PA - 3 office locations

THIS IS NOT A REMOTE POSITION.

Liken Home Care is hiring an assertive, highly organized, and technology-savvy Care Manager to grow and manage our private duty services. This role requires a confident professional who can influence decisions, coordinate multiple moving parts - someone who values health, wellness, and helping others succeed, while offering the opportunity to combine business development with meaningful, high-quality care.

Position Objective

The Care Manager is responsible for increasing new and existing business by delivering consistent quality service to consumers and referral sources. Success is measured by:

  • Growth in number of consumers served
  • Increase in billable hours
  • Strong retention and satisfaction of clients and caregivers

Key Responsibilities

Intake and Inquiry Management

  • Respond to incoming inquiries and requests for information
  • Educate and influence prospects to utilize Liken Home Care services
  • Convert inquiries into client intakes and active cases
  • Communicate regularly with consumers and their families throughout the care management process

Caregiver Assignment and Care Management

Work closely with the Care Team Coordinator to ensure the best match between caregivers and clients:

  • Search, match, and present assignments to qualified caregivers
  • Present caregiver options to clients and families
  • Schedule and coordinate client/caregiver meetings and interviews
  • Prepare caregivers for successful introductions
  • Conduct follow-up calls after meetings
  • Confirm schedules, shifts, and plans of care
  • Supervise first-visit assignments when needed

Quality Assurance and Customer Service

  • Perform ongoing care management checks including arrival confirmations
  • Conduct weekly, monthly, and quarterly quality reviews
  • Implement service recovery quickly and professionally when issues arise
  • Problem seek and solve to maintain consistent quality of service
  • Provide exceptional customer service to consumers, families, referral sources, and employees

Referral Source Relationship Building

  • Maintain close relationships with past, current, and potential referral sources
  • Conduct regular outreach via calls, e-mail, mail, and outside visits
  • Document all marketing and relationship activities in the company database
  • Enter and track new referral sources
  • Market qualified caregivers to partners as availability allows

Technology and Documentation

  • Maximize the use of scheduling and CRM technology
  • Maintain accurate records of inquiries, intakes, schedules, and changes
  • Enter data and prepare management reports in a timely manner
  • Provide regular feedback to leadership

Qualifications

  • Experience in home care, healthcare, staffing, or a related field - THIS IS NOT AN ENTRY LEVEL POSITION. Experience is a must.
  • Proven success in customer service and business development
  • Strong communication and organizational skills
  • Ability to influence decisions with professionalism and empathy
  • Skilled in scheduling, problem solving, and service recovery
  • Proficient with technology and database documentation
  • Management or supervisory experience necessary
