Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
Serious Illness Care- Team Lead
The Serious Illness Care (SIC) Palliative Care Team Lead is responsible for the day-to-day operational, clinical, and administrative oversight of the organization’s serious illness program. This individual ensures high-quality, patient-centered care delivery while coordinating staff schedules, streamlining workflows, supporting compliance, and maintaining financial and operational metrics. The Team Lead acts as a bridge between the clinical team and organizational leadership, fostering collaboration, quality improvement, and strategic growth.

Serious Illness Care- Team Lead

The Team Lead acts as a bridge between the clinical team and organizational leadership, fostering collaboration, quality improvement, and strategic growth.
Key Responsibilities:
- Spearhead efforts to achieve American Heart Association (AHA) certification, maintain strict compliance with federal/state regulations, and collaborate with Quality teams on policy development and audits.
- Oversee daily operations and personnel development for nurse practitioners and support staff, ensuring consistent clinical collaboration, effective onboarding, and team engagement.
- Partner closely with the Sr. Director to execute strategic initiatives, align office and field communications, and drive daily operational excellence.
- Monitor, analyze, and maintain key program metrics including census, productivity, visit frequency, response times, and patient satisfaction.
- Prepare and present performance reports to executive leadership, identifying trends and implementing data-driven strategies to optimize care delivery and operational efficiency.
- Facilitate communication between the SIC team and referring partners to ensure continuity and satisfaction of care.
- Registered nurse with current licensure to practice professional nursing in the state.
- Minimum of 2yrs leadership or management experience with direct experience in palliative, hospice or serious illness care is strongly preferred.
- Degree in Nursing, Healthcare Administration, Public Health, or related field preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
