What are the responsibilities and job description for the RN Team Support position at Lightways Hospice and Serious Illness Care?
Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
RN Team Support (Northeast Territory)
The RN Team Support plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Key Responsibilities:
- Conducts comprehensive patient assessments and develops, updates, and manages individualized care plans for hospice patients
- Provides skilled nursing care, including medication administration, treatments, and patient/family education
- Collaborates with the interdisciplinary team and participates in IDT meetings to ensure coordinated patient care
- Supports patient needs through on-call coverage, crisis response, and assistance with daily living activities
- Ensures compliance with regulatory standards, maintains accurate documentation, and meets quality and performance metrics
- Must be able to work 40 hours a week with a minimum of 20 hours on call/month, may increase based on staffing
Education/Experience:
- Current RN license
- Minimum of 2 years' experience, at least 1 year area of public health, home care, or hospice nursing is preferred
- Valid driver's license and insured, reliable vehicle

Salary : $70,000 - $110,000
