Summary: Uses specialized knowledge of human behavior and systems theory to assist families to improve their capacity to provide care to their children through direct service, counseling, interventions, and advocacy.

Essential Functions: 

1. Provides initial and ongoing assessment of clinical needs and document changes in level of care on a regular basis
2. Provides therapeutic services to clients and families who have been assessed as needing mental health intervention in order to stabilize symptoms of a mental health disorder.
3. Effectively engages clients, families and natural supports to participate in treatment through treatment planning and progress reviews.
4. Provides crisis intervention when needed.
5. Provides CPST services as needed such as linkage with other community resources, advocacy for clients with other agencies, coordination of services, emergency assistance, and assistance in basic living skills.
6. Initiates services within expected timeframe as set by agency standards.
7. Completes progress notes and service documentation per agency standards.
8. Maintains files in accordance with all regulatory compliance standards. 
9. Acts as liaison between referral sources, external partners and internal LYS service lines & programs. 
10. Provides back-up support and coverage to other employee and programs as needed. 
11. Participates in appropriate professional development activities and required training.

Supervisory Responsibility: This position has no supervisory responsibilities. 

Work Environment: This job operates in a professional office environment, but duties will also be carried out in a variety of settings including client homes, schools and in the community.  This job operates in a professional office environment, but duties will also be carried out in a variety of settings including client homes, schools and in the community.  This role has hybrid work options which involve both remote and in-office work. Specific in-office days will be determined in coordination with the team and the needs of the business.  This can change or be altered by the manager at any time.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms or communicate. The employee must occasionally lift and/or move up to 20 pounds.  

Position Type and Expected Hours of Work: Hours will be worked within standard hours of operation for assigned site. Occasionally the schedule may include evening, weekend or holidays hours to best meet the needs of clients and families. 

Travel: Travel is primarily regional during the week, although some out-of-the-area travel may be expected based on client needs. Transporting clients in personal vehicle is expected.

Required Education and Experience: 

1. Bachelor’s degree in social work, counseling or closely related field.
2. Professional licensure in State of Ohio as a Social Worker or Counselor. 
3. Excellent interpersonal and written communication skills. 
4. Demonstrated computer proficiency.

Preferred Education and Experience: 

1. Master’s degree in social work, counseling or closely related field.
2. Professional independent licensure in the State of Ohio as a Social Worker or Counselor. 
3. At least one year of professional social service experience.

Salary : $26 - $36
