Lighthouse Youth & Family Services is hiring for one full-time Case Manager Position in Community Based Residential Treatment. This position will provide case management services to youth at our residential treatment centers for boys and trans-youth between the ages of 11-17. The position requires a flexible schedule with the ability to work some evenings, as needed. The position also requires occasional travel to various client meetings and appointments.
Starting pay rate from $20.19
Competitive pay based on experience
Essential Functions:
- Provide oversight and management to assigned caseload of adolescent youth receiving treatment in a community-based residential environment.
- Assist the client in identifying areas of development to be successful at home, school and in the community.
- Advocate on client’s behalf to receive necessary treatment services and resources that will help to obtain ISP goals.
- Facilitate treatment team meetings to provide reports regarding client’s progress towards treatment goals. Identify barriers to client’s success and make adjustments to plan as necessary with client’s input.
- Attend team meetings, court appointments, school meetings, medication management appointments, etc.
- Thoroughly and consistently completes all client documentation in a timely manner, including progress notes, service tickets, intake packets and discharge summaries, after care summaries, etc.
- Complete intake and referral process for new residents of the program. Act as first point of contact for family, professionals, and members of treatment team
