What are the responsibilities and job description for the Case Manager position at LightHouse Social Service Centers?

POSITION SUMMARY:

The case manager provides housing case management services to participants. In addition, must demonstrates the ability to obtain and interpret information in terms of the participant’s needs, document knowledge of participant’s progress, and demonstrate an understanding of the range of services needed by the participant. Case Managers must enjoy working as a member of a team in a cooperative effort and be able to exercise creative and independent judgment to solve problems. Case Managers are expected to perform their duties in alignment with the vision and values of LightHouse Social Service Centers.

DUTIES AND RESPONSIBILITIES:

1. Conducts initial assessment and evaluation for the formulation of an Individual Housing Services Plan (IHSP).

2. Provides housing case management services.

3. Ensures both internal and collateral services are coordinated.

4. Assists participants in developing appropriate housing goals and makes revisions when necessary.

5. Participates in program planning.

6. Continues professional development, through participation in continuing education, in-service training and professional workshops to maintain current knowledge and broaden expertise.

7. Maintains participant records in a timely and appropriate manner.

8. Documents participant’s progress and reviews IHSP routinely.

9. Transports Clients

10. Conducts home visits

11. Complies with all policies and procedures.

12. Performs other related duties as assigned.

POSITION REQUIREMENTS:

a. BA or AA degree in Human Services preferred or two (2) years’ experience working in the field of homeless services or related human services experience.

b. Knowledge and experience with homeless communities and services.

c. Knowledge of community resources and services.

d. Strong computer and organizational skills.

e. Strong telephone skills.

f. Significant driving required.

g. If recovering, a minimum of two years sobriety is required.

h. Experience with culturally diverse populations.

i. Performs a flexible schedule that may include evenings.

j. Well-developed time management and conflict resolution skills.

k. Strong interpersonal skills; ability to work effectively with people in a constructive manner within a team setting.

l. Able to communicate verbally and writing is clear, precise and professional.

m. Valid driver’s license and insurable.

Job Type: Full-time

Pay: $27.50 - $28.50 per hour
