Registered Nurse (RN) - Hospice Case Manager (Full-Time)

Key Responsibilities

• Provide skilled nursing care, assessments, and interventions in alignment with hospice plans of care.
• Support patients and families with symptom management, education, and emotional support.
• Collaborate with physicians, social workers, chaplains, and other team members to ensure coordinated care.
• Document care and maintain accurate, timely clinical records per regulatory and agency standards.
• Participate in on-call rotation as needed.
• Advocate for patient dignity, comfort, and respect at all times.
• Attend biweekly IDG meetings.
