Job Summary
We are seeking a dynamic and experienced Clinical Manager - Registered Nurse to lead and oversee clinical operations within our Home Health Agency we are seeking an experienced Registered Nurse (RN) for the role of Clinical Manager. This position is designed to support the Director of Clinical Services in overseeing the clinicians employed by the agency. In addition to supervisory responsibilities, the Clinical Manager will maintain a part-time patient caseload. As the agency census increases, this patient caseload will be gradually reduced to allow more focus on management duties.
Responsibilities
- Oversee patient assessments, vital signs monitoring, and physical examinations to ensure accurate and thorough evaluations.
- Coordinate and participate in diagnostic evaluations including laboratory testing, wound care, med management, and catheters.
- Ensure proper documentation of medical records using EMR system AXXESS
- Monitor compliance with infection control protocols and implement best practices in aseptic technique to prevent healthcare-associated infections.
- Manage utilization review and utilization management processes to optimize resource use while maintaining high standards of care.
- Support continuous quality improvement initiatives aligned with NCQA standards and hospital accreditation requirements.
Requirements
- Valid Registered Nurse (RN) license with current state registration.
- Home Health: Proven experience providing nursing care in a home health setting.
- Supervisory: Prior experience in a leadership or supervisory role.
- Wound Care/Wound Vac: Skilled in managing wounds and utilizing wound vac therapy.
- Ostomy Care: Knowledgeable in ostomy care procedures and patient education.·
- Infusion Therapy: Competent in administering and monitoring infusion therapy.·
- Tracheostomy: Experience with tracheostomy care and maintenance.·
- OASIS: Proficient in completing OASIS assessments for home health patients.
- Excellent communication skills to collaborate with physicians, specialists in ophthalmology or cardiology, occupational health teams or discharge planning coordinators.

Base Pay: $75,000.00 - $85,000.00 per year
