Job Title: Teen Transition Program Instructor
Reports To: Program Manager
Status: Part-time/ Full-time possible
Location: Agency serves Pasco, Hernando, and Citrus Counties (main office at New Port Richey)
SUMMARY: Plan activities for Teen Transition Program (ages 14-22) including year-round activities two Saturdays a month and an intensive summer program focused on career exploration and skills. Activities should include activities in daily living skills development, use of and training in assistive technology and sensory based tools, career exploration, field trips to visit job sites, social activities, and recreational activities. Assist with planning, coordination and delivery of summer transition program. The position includes program coordination and case management for transition clients who are visually impaired and blind, curriculum development, developing and maintaining community contacts, and teaching with the team.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Work with the Lighthouse team to develop the plan of dates and activities for program participants.
- Develops curriculum and periodic reports for the Teen Transition Program.
- Provides direct services to clients in transition program, including those held on evenings and weekends.
- Ensures assessments are completed annually.
- Keep attendance log (using student sign in sheets).
- Document and report student progress (for AWARE database).
- Communicate with transportation coordinator to arrange transportation for students.
- Plan and lead two weekend activities a month during the school year for the students (may need to assist with transportation as well).
- Assist with planning, coordination and instruction in the summer transition program.
- Coordinates services with allied agencies.
- Acts as advocate for clients when necessary, including attending IEP’s in local schools.
- Provides case management and community referrals for teens and their families.
- Communicates with team regularly.
- Attends local and state meetings as necessary.
- Other duties as required or assigned.

COMPETENCY:
- Ability to work effectively and competently with teens and young adults.
- Strong communication skills, both verbal and written.
- Group leadership skills, including and understanding of group dynamics.
- Demonstrates organizational, creativity and event planning skills.
- Proficiency in computers and working knowledge of Microsoft Office software and email programs.
- Knowledge of the mission, objectives, policies, programs and procedures of the Lighthouse for the Visually Impaired and Blind; and the principles and practices of non-profit organizations.
- Demonstration of ability to communicate with different groups.
- Ability to function independently and with good judgement.
EDUCATION/EXPERIENCE:
- Prefer certification as Teacher of the Visually Impaired (TVI), Certified Vision Rehab Therapist (CVT), Certified Orientation & Mobility Specialist (COMS), or Certified Rehabilitation Counseling (CRC).
- Minimum of Bachelor’s Degree in related field (such as education, psychology, social work).
- A minimum of three years experience in human services or education.
- Experience working with the visually impaired people preferred.
- Case management experience preferred.
- Must be able to work flexible hours, including evenings and weekends.
- Must be able to successfully pass a criminal background screening and drug test.
