About the Role
We’re looking for a Social Care Network (SCN) Screener/Navigator who is drawn to this work for the right reasons — someone who understands that health isn’t just clinical, that trust is earned through follow-through, efficiency, and that the people we serve deserve support that is responsive, clear, and genuinely human.
In this role, you’ll be a first point of contact for Medicaid members who may be facing social and health-related needs — from food access and housing instability to care coordination and benefits navigation. You’ll screen for needs, connect people to eligible services, and stay in their corner as things move forward.
What You’ll Do
- Conduct Screenings — by phone or in-person— to assigned Medicaid members to initiate and maintain connection
- Use standardized screening tools in our designated Find Help, Channels 360 or Unite Us platform to assess members for health-related social needs (HRSNs) and connect them to eligible, available services
- Navigate across community-based organizations, service providers, and care teams on behalf of the people you serve
- Document all interactions thoroughly and on time using electronic health records and case management platforms, with attention to accuracy and confidentiality
- Apply culturally sensitive, strengths-based approaches in every client interaction — meeting people where they are, not where systems expect them to be
- Monitor progress, identify barriers, and follow through — because we don’t hand people off; we stay engaged
- Uphold HIPAA standards and all relevant privacy protections across all client interactions and records

Who You Are
- You have strong interpersonal skills and a natural ability to connect with people across backgrounds and life experiences
- You work quickly and efficiently — you manage your time well, move without needing to be pushed, and don’t let things sit
- You’re organized and accountable — you track what needs to be done and you do it
- You communicate clearly, in plain language, and you check that things landed
- You’re comfortable with technology — electronic records, Microsoft Suite, and virtual meeting tools
- You can write concise, objective, and accurate case notes
- You’re motivated by outcomes for people, not just completion of tasks
Qualifications
Required
- Associate’s Degree (any field)
- Proficiency in Microsoft Suite, and virtual platforms
- Strong organizational and written communication skills
Preferred
- Associate’s or Bachelor’s in health, human services, social work, or a related field
- 2 years of experience in care management, community health, or social services
- Experience with electronic health records or care management platforms
- Bilingual or multilingual candidates are strongly encouraged to apply
Compensation & Benefits
- Starting pay: $23.00 per hour
- Performance-based incentive structure — high performers can earn up to $38.00 per hour
- Generous paid time off (PTO)

Where & How You’ll Work
This is a hybrid role with office locations in Rockland County and Brooklyn, NY. You’ll work remotely or in person or a mix of both depending on caseload and program needs. Occasional community-based outreach may be part of the work.
