Role Description
This is a full-time on-site position for a Mental Health Case Manager based in Santa Clara, CA. The Mental Health Case Manager will be responsible for coordinating and managing individualized care plans for clients, providing case management services, maintaining accurate documentation, and collaborating closely with a multidisciplinary team. The role involves conducting client assessments, facilitating access to mental health resources, offering emotional support, and participating in treatment planning to support clients in achieving their recovery goals. Case Manager also facilitates IOP groups on topics related to resourcing and psychoeducation
Qualifications
- Experience in Social Work, Psychology, or a related mental health discipline
- Strong skills in Case Management and delivery of Case Management Services
- Excellent Interpersonal Skills for building and maintaining client and team relationships
- Ability to conduct assessments and develop individualized care plans
- Bachelor's degree in Social Work, Psychology, or a related field (Master's degree preferred)
- Knowledge of community resources and mental health support services
- Strong organizational, communication, and problem-solving skills
- Licensure or certification in social work, counseling, or related field is a plus
- Commitment to cultural competence and understanding of diverse communities
