Community Health Worker (CHW)

Under the direction of the Supervisor of Community Health & Wellness, the Community Health Worker (CHW) serves as a trusted link between the community and health/social service systems, helping improve access to care, reduce barriers, and promote whole-person wellness.
What You’ll Do
- Support individuals through in-office, in-home, and community-based outreach
- Connect consumers to community resources, benefits, and support services
- Conduct Social Determinants of Health (SDOH) screenings and follow-up support
- Assist with appointment scheduling, reminder calls, and wellness program participation
- Advocate for individuals and help improve access to behavioral health, substance use, and physical health services
- Provide encouragement, education, and recovery-focused support
- Facilitate communication between consumers and healthcare/social service providers
- Assist with wellness groups, health monitoring, and care coordination activities
Salary : $17 - $20
