The Case Manager supports consumers in accessing services and achieving personal goals. Responsibilities include assessment, planning, advocacy, coordination, and monitoring of health, housing, financial, employment, educational, and social services.
Key Responsibilities:
- Develop, implement, and monitor individualized plans of service (IPOS) using person-centered planning.
- Advocate for and coordinate services with providers, including healthcare and community supports.
- Assist consumers in accessing financial, medical, and other assistance programs.
- Support transitions (e.g., hospital to community, school to work) and crisis planning.
- Provide care coordination for consumers with court involvement (AOT/NGRI).
- Facilitate community inclusion, independence, and participation.
- Maintain regular attendance and perform duties in community settings, including consumer homes.
Salary : $50,807 - $63,340
