Job Title: Per Diem II - Direct Support Professional
Schedule: Per Diem (Flexible Scheduling, Minimum 16 Hours per Week)
Location: Various Locations Across Monroe County
Reports to: Workforce Management Coordinator

Lifetime Assistance, Inc. is seeking Per Diem II - Direct Support Professionals to join our team. This role provides direct care, support, and guidance to individuals with developmental disabilities, helping them live independently, build skills, and engage with their communities. Per Diem II DSPs possess additional certifications and training that allow them to meet a broader range of specialized needs.
What You'll Do:
- Provide personalized support, supervision, and training based on individuals' needs and service plans.
- Support individuals with daily living skills, including hygiene, meals, medication administration, and mobility assistance.
- Promote community integration by supporting meaningful activities and outings.
- Ensure safety and well-being at all times; report concerns promptly following agency policy.
- Accurately document all services and supports provided in the electronic health record system.
- Maintain required certifications, including SCIP-R, CPR/First Aid, Medication Certification, and Van Certification (as applicable).
- Follow individualized behavior support plans, nursing plans, and other person-centered documents.
- Assist in fostering independence, dignity, and positive social interactions.
- Maintain professionalism in communication, appearance, and behavior.
- Pick up shifts through the scheduling app or by coordinating availability with the Workforce Management Team.
- Attend trainings, meetings, and supervisor check-ins as scheduled.
What We're Looking For:
- Experience: Previous direct support or human services experience preferred.
- Certifications: SCIP-R, CPR/First Aid, Medication Administration Certification (or willingness to obtain), and Van Certification (as applicable).
- Skills: Compassion, patience, strong communication, teamwork, self-starter, and ability/capacity to work across programs/facilities.
- Commitment: Ability to meet minimum scheduling requirements (16 hours per week).
- Requirements: Background checks required.
