LifeSpring Health System’s Jeffersonville, IN office has an opening for a full-time Case Manager in our Children and Family Services program. Needed competencies include knowledge of children and families and their unique issues, the ability to teach ADL skills, and link clients to needed resources. A Bachelor’s degree in social work, psychology, education, or related field is highly desired; direct experience will be considered in lieu of education. Must be able to pass a DCS background check. We are looking for a caring, compassionate and energetic person who enjoy helping others. An electronic medical record is used so computer literacy is also required.
Responsibilities
- Conduct comprehensive assessments of children and families to identify mental health needs and develop individualized care plans.
- Provide ongoing support and case management services, ensuring adherence to treatment goals.
- Collaborate with healthcare providers, schools, and community resources to coordinate care and facilitate access to services.
- Maintain accurate and confidential records in compliance with HIPAA regulations.
- Educate families on mental health issues and available resources, fostering a supportive environment for recovery.
