QMHP Team Lead - Coordinated Specialty Care

Provides weekly, and as needed, supervision to Coordinated Specialty Care (CSC) staff under the supervision of the Program Administrator for Diversion Services. Ensures accurate data collection, sending in reports to Health and Human Service Commission (HHSC) as required, and to adhere to contractual fidelity requirements of the CSC program, including provision of minimum services hours by team (5 hours minimum). Performs field-based intensive social services work with eligible individuals aged 15-30, whom have had their first episode of psychosis within last 2 years. Responsibilities include providing routine case management, skills training & development, individual & family education, referral to appropriate state and federal benefits, supportive education and supportive employment services, & psychosocial rehabilitation to appropriate individuals. Ability to complete detailed, yet concise progress notes on all services provided during or immediately following the session. Position will work closely with various community collaborators and referral sources, including but not limited to: municipal police forces, court and jail liaisons, psychiatric & inpatient facilities, and other LifePath Systems personnel. Work closely with assigned CSC prescriber to ensure that prescriber schedule is maintained effectively. Goal is to increase awareness for individuals and their families of issues relating to psychosis, to ensure individuals receive the services they need to remain stable and functioning in the community. Must be able to work closely with all team members, and take initiative in completing complex tasks and projects, and other duties as assigned. Works under moderate supervision with limited latitude for the use of initiative and independent judgment.

Qualifications

Education, Training, & Experience (including licensure and certification)

  • Must have a Bachelor’s degree in psychology, social work or related field from a four year university
  • Bachelor’s degree must meet Texas Administrative Code requirements for a Qualified Mental Health Professional-Clinical Services (QMHP-CS)
  • Master’s Degree in counseling, social work, or related field preferred.
  • Current licensure in the State of Texas as an LPC, LCSW, or LMFT preferred.
  • Current licensure in the State of Texas as an LPC-Associate, LMSW, or LMFT-Associate considered.
  • Minimum of three years of related experience in the field of mental health for LPHA preferred.
  • Minimum of four years of related experience in the field of mental health for LPHA associate preferred.
  • Minimum of five years of related experience in the field of mental health for QMHP preferred.
  • Minimum of two years supervisory experience preferred.

Knowledge, Skills, & Abilities

  • Knowledge and experience in providing rehabilitative services to individuals with mental health concerns.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to write clearly and professionally.
  • Ability to think clearly and logically in order to deal with potentially dangerous and/or emotional situations.
  • Must have basic computer skills, ability to run reports, utilize excel effectively, and the ability to learn new software.
