Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, documenting vital signs and changes accurately in electronic health record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality
- Provide specialized care such as Alzheimer's and dementia care, memory care, hospice support, and end-of-life assistance with compassion and professionalism
- Operate patient transfer methods safely using equipment like Hoyer lifts and transfer techniques suitable for various mobility levels
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment
- Communicate effectively with clients, family members, and healthcare teams to ensure coordinated care plans and client satisfaction
Requirements
- Proven experience working with individuals with disabilities, geriatrics, or in assisted living or nursing home settings
- Knowledge of medical terminology, catheter care, vital signs measurement, medication administration, and basic life support (CPR/First Aid) certifications
- Familiarity with home health regulations, medical documentation practices, and EMR/EHR systems for accurate recordkeeping
- Ability to lift heavy objects safely and perform patient transfers using appropriate equipment such as Hoyer lifts or transfer methods
- Strong interpersonal skills with a caring attitude towards patients’ personal care needs and emotional well-being
- Experience working with clients who have developmental disabilities or conditions like Alzheimer’s disease or dementia is highly preferred
- Valid certification in CPR and First Aid; additional certifications in hospice or palliative care are a plus
- Must be reliable, energetic, detail-oriented, and committed to maintaining patient confidentiality under HIPAA guidelines
