POSITION OBJECTIVE: The role includes all facets of case management from initial assessment, identification of resident needs and case planning incorporating specialist rehabilitation and reintegration interventions and services, as well as facilitating links with the resident’s support groups and external providers. Central to the role will be the ability to exercise sound professional judgment to ensure that residents’ needs, and services are appropriately identified and provided, and that plan monitoring, review, and management is aligned to program and resident need.
POSITION DETAILS
Minimum 40 hours per week required; flexible between the hours of 8:00 am and 5:00 pm. On-site work assignment. This position is a full-time hourly position. Weekend flexibility/availability required for special projects or business needs required.
Pay ranges from $25 - $30 DOE
KEY RESPONSIBILITIES:
- Develop a written service plan with clients to meet client needs, assessing barriers to employment, and specifying goals to be achieved and services to be pursued.
- Coordinate and supervise care of clients. Provide direct service and support to individuals and clients, such as handling referrals and scheduling appointments.
- Reassess and monitors client situation and adequacy of service plan.
- Assess readiness for employment utilizing motivational interviewing.
- Works one-on-one with clients to address issues that arise during the program that threaten employability and refer to appropriate services as needed.
- Coordinate with outside referral resources and outreach services. Establish and maintain positive working relationships with other agencies and organizations in the community to ensure clients' needs are met.
- Take increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
- Maintain records in accordance with agency, government, other funding sources, accreditation, and/or licensing requirements.
- Work effectively with diverse staff and service population.
- All other duties as assigned.
REQUIRED SKILLS:
- Knowledge of general principles and approaches for providing services to clients experiencing housing insecurity, substance addiction, interpersonal trauma, and criminal justice involvement.
- Must be empathetic, patient, and skilled at working with combat veterans, families, youth, and single men. Must have ability to remain calm in crisis, hostile, or disaster situations.
- Familiar with shelter, public housing, and group living dynamics.
- Possess excellent interpersonal skills and ability to support clients through difficult and emotionally complex situations.
- Knowledge of MS Office applications and various EHS systems is a plus.
MINIMUM QUALIFICATIONS:
- Bachelor’s (Preferred) or a combination of education and experience preferably in social service work
- US work authorization (Required)
- Case management or housing navigation within a social service or healthcare setting: 2 years (Preferred)
- Reliable Transportation
- Excellent Communication necessary
- Demonstrated ability to provide adequate service to clients and to organize and manage job efficiently

PHYSICAL DEMANDS:
Ability to sit at a desk for several hours, use a computer, climb stairs, potential local travel. ability to respond to an emergency such as fire drills by going through the building in less than three minutes to ensure that occupants have exited.
