What is a Host Home Provider?

A Host Home Provider welcomes an adult with a developmental disability to live their home and supports them as part of everyday life. The individual lives with you in your home, apartment, or condo and becomes part of your household. Providers help create a stable and supportive environment where the person can build independence, develop life skills, and participate in their community. LifeConnections specializes in supporting individuals with complex behavioral needs, as well as medical complexities. Our team has extensive experience providing the structure, training, and professional guidance necessary for success for both the provider and individual.

Host Home Providers are never expected to manage challenging situations alone. Our clinical and program teams provide:

  • Specialized training
  • Behavioral guidance and coaching
  • Ongoing support from experienced staff
  • Regular home visits and check-ins

Our goal is to ensure that both the individual and the provider feel supported, confident, and successful!

  • Caregivers or CNAs
  • Special education staff
  • Former foster parents
