What are the responsibilities and job description for the Targeted Case Manager position at Life Works?
We are looking for a Targeted Case Manager to enhance the quality of life for IDD/MI client/student management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full client/student care cycle from intakes to the closing of files. Case Manager duties include intake meetings, assessing, planning, good writing skills and grammar, implementing, monitoring files, writing and creating PCSP's, Behavior Support Plans, and evaluating actions required to meet the client’s resource needs and human service's needs. A certified case manager is a plus. To qualify for this position the candidate must have a college degree or 6 months experience for every semester of college.
Responsibilities
- Coordinate plans that provide safety, timely, effective, efficient, equitable, and client-centered
- Handle caseload, draft PCSP and BSP, review case progress and determine case closure if needed, create case notes for billing
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help clients/students make informed decisions by acting as their advocate regarding their status
- Develop effective working relations and cooperate with the team throughout the entire case management process
- Arrange abeyances with social services, health and governmental agencies if needed. If not, know how to refer parents/ guardians to this recourse.
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction. You can do this during your visits with the client/student
- Record cases information, complete accurately all necessary forms
- Promote quality and interventions and outcomes
- Assess and address motivational and psychosocial issues. This may be referring to a recourse.
- Adhere to professional standards as outlined by protocols, rules and regulations
Skills
- Proven working experience in case management, or a related job.
- Excellent knowledge of case management principles, social service system, management, etc.
- College degree or 6 months experience for every semester of college
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
