As a Primary Treatment Coordinator (PTC), you’ll be part of a dedicated team that offers a full spectrum of services, including Residential Living, Case Management, Individual Therapy, Substance Use Education, Relapse Prevention, Family Support, and Aftercare.
This role is perfect for an energetic self-starter who thrives in a people-centered environment. Comparable roles might include substance use disorder (SUD) counselor, mental health counselor, social worker, or case manager but here you will play a key role in helping individuals overcome challenges related to addiction and work toward lasting recovery. You’ll provide evidence-based counseling services, develop individualized treatment plans, and guide persons served through each stage of their recovery journey. This role combines clinical expertise with compassion, ensuring persons served feel supported, understood, and empowered to make positive changes.

Responsibilities
- Conducting assessments and creating individualized treatment plans tailored to each persons served needs
- Facilitating individual, group, and family counseling sessions to provide therapeutic support and education
- Teaching relapse prevention strategies and healthy coping skills to empower persons served in their recovery journey
- Monitoring persons served progress and adjusting treatment plans as necessary to ensure positive outcomes
- Connecting persons served with additional support services, including medical care, social services, and employment resources
- Collaborating with the treatment team, including case managers, housing staff, and other professionals to coordinate comprehensive care
- Maintaining accurate and timely clinical documentation and preparing required reports
- Preparing court-related documentation and providing support by accompanying persons served to court when needed
- Providing education and guidance to persons served and their families about addiction, recovery, and available community resources
- Participating in on-call rotations as assigned to ensure persons served support outside standard hours
Qualifications
- Master’s Degree in Mental Health Counseling, Addictions Counseling, Marriage and Family, Psychology, Pastoral Counseling, or Social Work (required)
- Active LSW, LCSW, LMHCa, LMHC, LCAC, LAC, LMFT, or Psychology License in Indiana (preferred)
- Valid Driver's License
