Hospice RN Case-manager
Our Hospice RN Case-managers are responsible for providing and documenting care for terminally ill patients from day of admission through the day of discharge. Utilizing physicians’ orders and professional skills, the hospice nurse implements a hospice plan that meets each patient’s specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures.
The position is working with patients in their home or nursing home setting.
2 years of RN experience required. Previous hospice, home health, or nursing home experience a plus.
