Position Overview
We are seeking a Registered Dietitian to join our growing team. The ideal candidate will provide nutritional assessments, counseling, and education to patients across diverse settings. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in the community.
Key Responsibilities
- Conduct comprehensive nutritional assessments for patients in private homes and assisted living facilities.
- Develop individualized nutrition care plans based on medical history, dietary preferences, and health goals.
- Provide ongoing counseling and education to patients, families, and caregivers.
- Collaborate with physicians, nurses, and other healthcare providers to ensure coordinated patient care.
- Monitor patient progress and adjust care plans as needed.
- Maintain accurate documentation in compliance with company and regulatory standards.
Qualifications
- Current licensure as a Registered Dietitian (RD/RDN) in New Jersey.
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Experience in home health or assisted living settings preferred but not required.
