Role OverviewWe are seeking a skilled and empathetic Nurse Practitioner (NP) or Physician Assistant (PA) with experience in palliative care to join our mobile medical team. This role is ideal for a clinician who values autonomy, meaningful patient relationships, and interdisciplinary collaboration.

Responsibilities

• Provide comprehensive palliative care to patients with chronic or life-limiting illnesses
• Conduct in-home assessments, develop care plans, and manage symptoms
• Collaborate with physicians, caregivers, and facility staff to ensure continuity of care
• Educate patients and families on disease progression, treatment options, and end-of-life planning
• Document visits and maintain accurate medical records in compliance with regulatory standards
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives

Qualifications

• Active NP or PA license in the state of New Jersey
• Certification in palliative care or relevant experience strongly preferred
• Minimum of 2 years clinical experience in geriatrics, hospice, or chronic disease management
• Strong communication and interpersonal skills
• Ability to work independently and travel to patient locations
• Reliable transportation and valid driver’s license
