TITLE OF POSITION: REGISTERED NURSE- ON CALL

Position Summary: We are seeking a dedicated On-Call Hospice RN to join our team. This role is ideal for a nurse who thrives in a flexible, autonomous environment. The nurse will be on-call for 7-days, followed by 7 days off. During the on-call period, the nurse is responsible for triaging calls, making home visits when necessary, and providing critical support to patients and families during after-hours.
KEY RESPONSIBILITIES
- Respond to patient calls and triage health concerns appropriately
- Provide clinical guidance over the phone or in person as needed
- Coordinate with physicians and care teams for follow-up or escalation
- Maintain accurate documentation of patient interactions
- Support emergency response protocols when necessary
- Provide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decisions
- Perform death visit per agency’s policy

PAY RATE: 93,000-95,000

QUALIFICATIONS
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in NJ as a Registered Nurse by New Jersey State Board of Nursing in accordance with NJSA 45:11-23 et seq and NJAC 13:37.
- One year of RN experience. Hospice experience preferred.
- Must have excellent assessment skills and problem-solving skills.
- Should be skillful in organization and in the principles of time management and have knowledge of case management processes.
- Must be a licensed driver with an insured automobile in good working order.
- The ability to communicate well, both verbally and in writing.
- Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state. All skills to be competencied before actual hands-on patient care, and when procedures or treatments are not usually performed.
