Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary.
- Monitor and record vital signs accurately, noting any changes in patient condition and reporting them promptly to the supervising nurse or healthcare team.
- Provide medication administration under proper protocols, ensuring timely and safe delivery of prescribed treatments.
- Support patients with specialized care needs such as Alzheimer's disease, dementia, memory care, and hospice-related comfort measures.
- Perform patient observation to identify signs of distress or deterioration and document medical records in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining HIPAA compliance.
- Assist with catheter care, wound management, and other medical procedures as trained and authorized.
- Support families by providing education on care techniques, safety measures, and emotional support during difficult times.
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation to promote overall well-being.
Skills
- Strong knowledge of medical terminology and home health care practices.
- Experience working with people with disabilities, developmental disabilities, or in assisted living/nursing home environments.
- Proficiency in using EMR/EHR systems for documentation and medical records management.
- Ability to perform patient observation accurately and respond swiftly to changes in condition.
- Certification in CPR is required; additional certifications such as hospice care training or dementia/caregiver certifications are a plus.
- Familiarity with medical equipment including Hoyer lifts, blood pressure cuffs, thermometers, and other vital signs monitoring tools.
- Excellent communication skills to collaborate effectively with healthcare teams and families while maintaining confidentiality per HIPAA standards.
