Responsibilities
- Deliver comprehensive assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ health status by recording vital signs like blood pressure, pulse, temperature, and respiration rate
- Assist with medication administration following prescribed protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Support patients with specialized needs including Alzheimer’s care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities
- Operate medical equipment such as Hoyer lifts for safe patient transfers and catheter care devices as needed
- Provide meal preparation and nutritional support tailored to dietary restrictions and preferences
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and safety
- Document all patient interactions thoroughly in compliance with HIPAA regulations to ensure confidentiality and proper medical records management
- Collaborate effectively with healthcare teams to coordinate patient care plans and communicate any changes in condition promptly
- Assist in emergency situations by performing CPR when necessary and following established protocols for patient safety
Skills
- Strong ability to observe and assess patient conditions accurately through patient observation skills
- Experience working in assisted living facilities, nursing homes, or home & community care environments
- Proficiency with EMR/EHR systems for documentation and medical records management
- Knowledge of medical terminology relevant to home health care settings
- Certified in CPR; additional certifications such as hospice care training or dementia certification are advantageous
- Skilled in assisting with ADLs including bathing, dressing, grooming, mobility support, and medication administration
- Familiarity with medical devices like Hoyer lifts and catheter care procedures
- Compassionate caregiver with excellent communication skills working effectively with people of all ages and abilities
- Ability to handle sensitive situations discreetly while maintaining HIPAA compliance
- Experience working with people with disabilities or developmental challenges is preferred
- Capable of performing light housekeeping tasks such as cleaning, laundry, meal prep, and ensuring a safe environment for clients
