This is a full-time on-site role as an Admission Nurse at Life Hospice, located in Livingston, NJ. The Admission Nurse will be responsible for evaluating and admitting new patients to hospice care, conducting initial patient assessments, creating personalized care plans, and coordinating with other healthcare professionals to ensure consistent and effective patient care. Additionally, the Admission Nurse will provide palliative care, educate patients and families about the hospice process, and ensure compliance with all relevant regulations and standards.
- Nursing and Medicine skills
- Experience in Palliative Care
- Home Care and Long-term Care skills
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work independently and as part of a team
- Current nursing license in the state of New Jersey
- Prior hospice or home health experience is a plus
Salary : $50 - $52
