Position Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) Case Manager to join our team. The RN Case Manager is responsible for coordinating and managing the care of hospice patients, ensuring that patients and their families receive high-quality end-of-life care. The ideal candidate will be comfortable working independently in patients’ homes and facilities while collaborating closely with the interdisciplinary hospice team.
Key Responsibilities:
- Manage and coordinate the care of an assigned hospice patient caseload.
- Conduct initial patient assessments and develop individualized care plans.
- Provide skilled nursing care to hospice patients in homes, nursing facilities, and assisted living communities.
- Monitor patient condition and adjust care plans as needed.
- Educate patients and family members on disease progression, symptom management, and end-of-life care.
- Coordinate care with physicians, social workers, chaplains, home health aides, and other members of the interdisciplinary team.
- Ensure timely and accurate documentation in the electronic medical record system.
- Participate in interdisciplinary team meetings and care planning discussions.
- Provide emotional support and guidance to patients and families during the hospice journey.
- Conduct HHA and LPN supervisory visits
Qualifications:
- Active Registered Nurse (RN) license in the State of New Jersey.
- Minimum 1–2 years of nursing experience required.
- Hospice or home health experience preferred but not required.
- Strong assessment and critical thinking skills.
- Ability to work independently and manage time effectively.
- Excellent communication and interpersonal skills.
- Reliable transportation and valid driver’s license.

Qualifications:
QUALIFICATIONS
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in NJ as a Registered Nurse by New Jersey State Board of Nursing in
- accordance with NJSA 45:11-23 et seq and NJAC 13:37.
- One year of RN experience. Hospice experience preferred.
- Must have excellent assessment skills and problem-solving skills.
- Should be skillful in organization and in the principles of time management and have knowledge of case management processes.
- Must be a licensed driver with an insured automobile in good working order.
- The ability to communicate well, both verbally and in writing.
- Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state. All skills to be competencies before actual hands-on patient care, and when procedures or treatments are not usually performed.
